Certified by Doreen Virtue, PhD; Master Practitioner of Neuro-Linguistic
Programming; Time Line Therapy TM Practitioner; and practitioner of
The Second Degree of The Radiance Technique®/TRT®/Authentic Reiki®.
These are not medical designations, and are not regulated or licensed
by the state or federal government. A Hypnotherapist is not a licensed
Healing Arts Practitioner.
The sessions offered by Claire Peterson Jacks are for vocational and
avocational self-improvement (California Business and Professional Code 2908). This
work is not to take the place of psychological or medical care. Problems
of medical or psychological origin need to be addressed by the
appropriate doctors/professionals. By referral, this program can be used
as an adjunct to medical or psychotherapeutic care (California Business and
Professional Code 2908).
This self-improvement program includes an undetermined number of
private sessions. There are no guarantees as to the results or progress
that will be made.
While it is the practice of hypnotherapists to keep information
confidential, information revealed in hypnotherapy is not subject to
the psychotherapist-patient privilege. A court may order disclosure of
information learned in therapy.
The client agrees to give at least 24 hours notice to cancel or change
scheduled appointments. If the client misses a scheduled appointment
without giving 24 hours notice, the client may be charged for the
appointment. The client agrees to make payment in full at the end of
each session.
